Septal Defect and Severe Pulmonary Hypertension

چکیده
SUCCESS in the surgical treatment of children who have large ventricular septal defects is dependent in part on the proper selection of patients for operation. The patients most deser-ving of such correction are those who have severe pulmonary hypertension, and it is in these that the decision to offer surgical intervention may present the greatest problems. The factors influencing the increase in pulmonary arterial pressure deserve critical analysis. Elevated pulmonary arterial pressure in patients with large ventricular septal defects is, to a great extent, the result of 2 factors, namely increased pulmonary blood flow or increased pulmonary vascular resistance; both factors are present in some instances. -in those patients without pulmonary vascular disease but with a high pulmonary blood flow in relationship to systemic blood flow and witlh only slightly increased pulmonary vascular resistance, the pulmonary arterial pressure may be equal or nearly equal to the systemic pressure. In those who have complicating pulmonary vascular disease in whom the pulmonary resistance is increased and the pulmonary blood flow is not increased in relationship to systemic flow, the pulmonary and systemic pressures may be equal or nearly equal. If the increased pulmonary arterial pressure is prinmarily the result of increased pulmonary blood flow from a left-to-right shunt, surgical closure of the defect will result in a decrease in pulnmonary arterial pressure. Integrated Percutaneous Atrial Septal Defect Occlusion and Pulmonary Balloon Valvuloplasty
Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...
متن کاملAtrial septal defect with severe pulmonary hypertension in elderly patients: usefulness of transient balloon occlusion.
In patients with an atrial septal defect and severe pulmonary hypertension, it is important to determine whether the latter is reversible before percutaneous or surgical closure. In addition to determining pulmonary resistance, one simple technique is to transiently occlude the septal defect using a balloon catheter and to evaluate the hemodynamic response. We defined a positive response as a >...
متن کاملLarge Atrial Septal Defect Closure in a Patient with Severe Pulmonary Arterial Hypertension
Patients with an atrial septal defect (ASD) and severe pulmonary arterial hypertension (PAH) are considered ineligible for defect closure surgery because of the risk of right ventricular decompensation and death after the operation. We report the case of a patient with large ASD and severe PAH who was able to undergo defect closure surgery successfully following long-term use of combined oral s...
متن کاملSmall ventricular septal defect associated with severe pulmonary hypertension.
A small ventricular septal defect in an 18 year old man was shown by serial cardiac catheterisation to be associated with a progressive increase in pulmonary arterial pressure. Surgical closure of the defect had no effect on the increased pulmonary arterial pressure. Whether pulmonary hypertension occurred secondary to the haemodynamics of the ventricular septal defect or as an independent even...

We report a woman with atrial septal defect and severe pulmonary hypertension with 25.0 Wood unit.m(2) of indexed total pulmonary vascular resistance. She underwent successful corrective repair of atrial septal defect after 2 years of treatment with sildenafil, and has been monitored for 4 years after repair. This case supports a "treat and repair" approach using advanced pulmonary vasodilator ...
